Providing home care services continues

CHCA continues to provide home care services to 25 people in Zugdidi Municipality.

Beneficiaries involved in the home care service provision sub-program are supported by a multidisciplinary team:
-Coordinator
-Doctor
-Nurse
-Social worker
-Caregiver
-Technical assistant

They conduct a detailed assessment of the medical, social and household needs of the beneficiaries, draw up an individual plan and provide home care services in accordance with the home care standard, program and the beneficiary's individual plan.

The process includes:
- Assessment of the beneficiary's physical and mental state,
- Monitoring of vital parameters,
- Provision of medications,
- Social support,
- Basic care,
- Support in daily activities.

The goal of the home care program is to improve the quality of life of people in need of care, in particular, to maximize the independence of beneficiaries in their daily activities in their usual living conditions, with respect for ethical norms.
