CHCA provides home care services to 25 people in Zugdidi Municipality.

Beneficiaries in need included in the Home Care Services Sub-program are served by a multidisciplinary team: coordinator, doctor, nurse, social worker, caregiver, technical assistant.

The beneficiary of the home care program is a citizen aged 18 and over who belongs to the category of severely, significantly or moderately dependent and requires supervision and assistance from a caregiver and other members of the home care multidisciplinary team.

The team conducts a detailed assessment of the beneficiary's medical, social and household needs, draws up an individual plan and provides home care services in accordance with the home care standard, program and the beneficiary's
individual plan.

The process includes: assessing the user's physical and mental condition, monitoring vital parameters, providing medication, social support, basic care, and support in daily activities.
